POSITION 1: Residential Coordinator

Position Category: Middle Management

Scope: The Residential Coordinator reports directly to the Director of Residential Services. The Residential Coordinator is responsible for the day-to-day functioning of the assigned community homes.

Basic Function: The Residential Coordinator is primarily responsible for the overall functioning of the assigned community homes. The Residential Coordinator will supervise the Direct Support Professionals and Residential Supervisors within the assigned community homes, as well as the daily activities that occur in the community home. The Residential Coordinator will function as an active member in the interdisciplinary team (IDT), as well as serve as a role model for all staff and residents within the assigned community homes.

Position Schedule: The Residential Coordinator position is a full-time position that requires on-call duties for emergency coverage. There is a rotating on-call duty associated with this position.

Position Qualifications: Bachelor’s Degree in Human Services or related field and (2) two years’ experience working with individuals with intellectual and/or developmental disabilities OR Associate’s Degree in Human Services or related field and (4) four years’ experience working with individuals with intellectual and/or developmental disabilities. Previous supervisory experience is preferred for this position.

Essential Job Elements:

I. The Residential Coordinator will plan, develop, implement, organize and document programs for their assigned residential caseload.
II. The Residential Coordinator, in conjunction with the Residential Supervisor, will develop and implement a shift responsibility list for daily household tasks.
III. The Residential Coordinator will assist in coordinating, as well as participating in, the transportation for residents’ health appointments, as well as community engagement activities on an as-needed basis.
IV. The Residential Coordinator will develop goal plans, including long-term goals, for their assigned residential caseload.
V. The Residential Coordinator in conjunction with the Residential Supervisor, will be responsible for implementing the residents’ Individual Support Plan (ISP) as developed by the person-centered planning team.
VI. The Residential Coordinator will ensure that daily documentation is accurately documented by all staff within their assigned community homes.
VII. The Residential Coordinator will ensure the residents’ funds are accurately accounted for and documented at each shift exchange.
VIII. The Residential Coordinator will participate in approved safety techniques in accordance with the agency’s philosophy and practices.
IX. The Residential Coordinator will report any maintenance concerns to the Residential Director and Community Sites Manager.
X. The Residential Coordinator will maintain open communication with the residents in their assigned community home. This is inclusive of receiving input from the residents regarding their home and activities of their interest for program planning.
XI. The Residential Coordinator will assist the residents with their activities of daily living inclusive of personal care, meal preparation/serving, medication administration, scheduling and attending medical appointments, etc. if emergency coverage is needed.
XII. The Residential Coordinator will ensure a home-like atmosphere within their assigned community homes amongst all staff, as well as the residents who reside in the assigned community homes.
XIII. The Residential Coordinator, in conjunction with the Residential Supervisor, is responsible for the immediate health and safety of the community home’s residents.
XIV. The Residential Coordinator will audit monthly fire drills, ensuring all applicable documentation is accurately and consistently documented and completed.
XV. The Residential Coordinator will be responsible for developing schedules in the absence of a Residential Supervisor, for the applicable community home, ensuring applicable staffing ratios are upheld at all times.
XVI. The Residential Coordinator will be flexible to the needs of the program, and will adapt to changes in duties, methods, priorities and environments.
XVII. The Residential Coordinator will maintain a cooperative relationship with staff, supervisors, and Directors, and will work to create an atmosphere of mutual respect and trust within the facility.
XVIII. The Residential Coordinator will be responsible for the development of the Individual Support Plan (ISP), attending the annual ISP Update Meetings, as well as any and all ISP revisions in compliance with Chapter 6400 Regulations.
XIX. The Residential Coordinator will assume responsibility of reporting any content discrepancies to the appropriate Supports Coordinator and/or planning team members.
XX. The Residential Coordinator will support the integration of individuals in the community, providing support and supervision of activities for the individuals in accordance with the individual plans.
XXI. The Residential Coordinator will provide positive and constructive feedback to all staff within the assigned community homes, ensuring quality services that meet and exceed expectations of the agency.
XXII. The Residential Coordinator will both support, as well as promote, the philosophy of UCP of NEPA and the residential department’s policies and procedures effectively and consistently.
XXIII. The Residential Coordinator will assist in the training of all new staff, and will act as a role model for Supervisors and Direct Support Professionals.

Physical Demands

  1. The physical duties that are required of an employee in this position include the ability to effectively drive a program vehicle, operate a handicap wheelchair lift, push wheelchairs, transfer and/or position residents, walk up and down flights of stairs, carry laundry baskets, grocery bags, assist in routine household chores, as well as participate in training and the implementation of safety techniques as required by the UCP of NEPA, State, and Federal requirements.

Health Considerations

  1. In working in the client population, there is a risk of exposure to Hepatitis B or other infectious and/or communicable diseases. Staff are informed of the risk, as well as necessary precautions, and staff are responsible to adhere to the agency’s policy/practices regarding health and safety standards.
  2. Staff are offered the Hepatitis B vaccine upon employment with UCP of NEPA at no personal cost. Staff may decline the vaccine opportunity, but may request to receive the vaccine at any time during employment with UCP of NEPA. Staff who decline the vaccine opportunity are required to document their refusal at the time of the offer.

POSITION 2: Residential Supervisor

Position Category: Frontline Management

Scope: The Residential Supervisor reports directly to the Residential Coordinator. The Residential Supervisor is responsible for the day-to-day functioning of the assigned community home.

Basic Function: The Residential Supervisor is primarily responsible for the overall functioning of the assigned community home. The Residential Supervisor will supervise the Direct Support Professionals within the assigned community home, as well as the daily activities that occur in the community home. The Residential Supervisor will function as an active member in the interdisciplinary team (IDT), as well as serve as a role model for the Direct Support Professionals within the community home.

Position Schedule: The Residential Supervisor is a full-time, 40 hours per week position that requires on-call duties for emergency coverage. During the Residential Supervisor’s days scheduled, this position requires the Supervisor to be on-call to cover shifts as needed.

Essential Job Elements:

I. The Residential Supervisor will plan menus on a monthly basis within the limits of the community home’s food budget.
II. The Residential Supervisor, in conjunction with the Residential Coordinator, will develop and implement a shift responsibility list for daily household tasks.
III. The Residential Supervisor will coordinate, as well as participate in, the transportation for residents’ health appointments, as well as community engagement activities.
IV. The Residential Supervisor will complete a semi-annual inventory of residents’ personal possessions and clothing. This inventory will remain updated in between each semi-annual inventory revision.
V. The Residential Supervisor, in conjunction with the Residential Coordinator, will be responsible for implementing the residents’ Individual Support Plan (ISP) as developed by the person-centered planning team.
VI. The Residential Supervisor will ensure that daily documentation is accurately documented by all staff within the assigned community home.
VII. The Residential Supervisor will ensure the residents’ funds are accurately accounted for and documented at each shift exchange.

POSITION 3: Direct Support Professional

Basic Function: A Direct Support Professional (DSP) is a professional position that is primarily responsible for supporting individuals with intellectual and/or developmental disabilities (IDD) in a community home residence.

Job Description:

  • A DSP will support the individuals in the assigned community home with their activities of daily living to include, but not limited to, personal care, medication administration, housekeeping, preparing and/or serving meals, coordinating activities within the community home, participating in community engagement activities of the individual's choice, transportation to and from activities/medical appointments/day and/or vocational program services.
  • A DSP will demonstrate and be able to perform monthly fire drills within their assigned community home.
  • A DSP will work collaboratively with their co-workers and the individuals in the assigned community home to ensure a positive, nurturing home-like atmosphere.
  • A DSP will report any safety and/or maintenance concerns to the Residential Supervisor and Residential Coordinator.
  • A DSP will be responsible for implementing the Individual Support Plan (ISP) as prepared by the person-centered planning team.
  • A DSP will demonstrate the ability to reinforce the philosophy of the agency, as well as demonstrate basic knowledge of the PA Chapter 6400 regulations for licensed community homes.
  • A DSP must complete (24) twenty-four hours of in-service training, inclusive of fire safety training annually. Trainings are provided by the agency.

All Direct Support Professionals will receive paid training, CPR/First Aid training, program vehicle trainings, and other trainings as required by Local, State and Federal requirements.

Shifts: Full time, part time (days, evenings, weekends) 

Salary: $18.80**/ hour

POSITION 4: DSP Float

Shifts are as follows:

7 a.m. to 3 p.m.

  • Tuesday/Wednesday/Thursday/Friday/Saturday - FULL TIME (40 hrs./week)
  • Sunday/Monday - PART TIME (16 hrs./week)

3 to 11 p.m.

  • Tuesday/Wednesday/Thursday/Friday/Saturday - FULL TIME (40 hrs./week)
  • Sunday/Monday - PART TIME (16 hrs./week)

11 p.m. to 9 a.m.*

  • Sunday/Monday - PART TIME (20 hrs./week)

Pay Rate: $18.80*/ hour; $19.57 for awake, overnight shift

Basic Function: A Direct Support Professional (DSP) Float is a professional position that is primarily responsible for supporting individuals with intellectual and/or developmental disabilities (IDD) in a community home residence. The DSP Float will be trained in all of UCP of NEPA's community homes to fill in vacancies on an ongoing basis.

Job Description:

  • A DSP will support the individuals in the assigned community home with their activities of daily living to include, but not limited to, personal care, medication administration, housekeeping, preparing and/or serving meals, coordinating activities within the community home, participating in community engagement activities of the individual's choice, transportation to and from activities/medical appointments/day and/or vocational program services.

  • A DSP will work collaboratively with their co-workers and the individuals in the assigned community home to ensure a positive, nurturing home-like atmosphere.

  • A DSP will be responsible for implementing the Individual Support Plan (ISP) as prepared by the person-centered planning team.
